It can happen either way. Generally the parties will meet separately at the mediator’s discretion. When you meet separately, it’s called caucusing. One party and their attorney are in one room, the other party and their attorney are in another room, and the mediator goes back and forth to try and work out a settlement agreement. However sometimes the parties and/or the mediator feel that it is in the best interests of the negotiation process to meet all together collectively in one room, which can be done.
